NHS Grampian fears an Inverurie GP would struggle with an influx of new patients if 49 new Port Elphinstone homes are approved.
Places for People Scotland is behind a proposal to redevelop the empty Blythewood House site on School Road.
It wants to construct 23 houses, 24 flats and two self-contained cottage flats on the vacant land.
But the project has been met with concern from the health board.
NHS Grampian submitted a letter to Aberdeenshire Council raising fears that the development could negatively impact local services.
In the document, health bosses noted the development would create at least 113 extra patients for Inverurie Medical Practice.
It said: “The practice has limited remaining capacity compared to its floor area allowance.”
But, it suggested that it would seek developer contributions from the project to help increase capacity at the Upperboat Road facility.
The letter also stated the Aberdeenshire Health and Social Care Partnership had “significant concerns” about staffing concerns in the area.
It added that the partnership “wish it to be known that staffing of NHS services is very challenging in the current financial climate”.
If approved, the homes would bring the vacant site of the former Blythewood House care come back to life.
It was closed in 2016 and later destroyed by a fire in 2018.
But now a mix of social rent and mid-market rent houses and apartments could be constructed on the site.
The development would include 23 houses ranging from two to four bedrooms in size.
Meanwhile, three and four-storey blocks of one and two-bedroom flats would also be added.
Footpaths would be formed throughout the development to make it more accessible to residents and link it to the surrounding area.
